If your company is paying for your test we must have an authorization form to begin your drug test. 

If you are paying for your test you do not need an authorization form 

An authorization includes which test to do, which lab to send to, where to send results, who is paying  etc.

We follow DOT standards on all collections – All of our collectors are DOT certified
