Do I have to take a drug test for the DOT exam?
No, the urine sample provided at the DOT exam is to check for glucose, protein, blood and specific gravity.

Do in need to bring my healing aids to my DOT exam?
Yes, if you don’t being your hearing aids to the DOT exam other wise you will be disqualified and have to pay to take the exam again when you have your hearing aids.

Do I need to bring my glasses or contacts my DOT exam?
Yes, if you don’t bring your glasses or contacts you will be disqualified and have to pay to take the exam again when you have your glasses or contacts.

What forms of ID are acceptable?
Government issued ID including drivers license, passport, military card or tribal cards are accepted.

Do I have to bring my Cpap or Bipap report?
Yes, you need to bring a current 90 day Cpap or Bipap report showing that you use the device more than 4 hours a night 70% of the time.

Do I need to bring additional forms if I use insulin?
